Claim Form

All causes of loss must be due to a naturally occurring event with the exception of a loss in revenue covered in the Crop Revenue Coverage (CRC) Plan. Crop Insurance Policy requires prompt notification in the event of a loss:

  • Preventive Planting
      • 72 hours notice after Final Plant Date of crop
      • 72 hours notice after determining inability to plant crop within applicable late planting period of crop
  • Crop damage – 72 hours notice
  • Pre-harvest – 15 days notice
  • Post-harvest – 15 days notice
  • No later than the End of Insurance Period:
      • Pecans – January 31st
      • Onions – June 1st
      • Wheat – July 31st
      • Blueberries – September 15th
      • Peanuts – November 30th
      • Soybeans – December 10th
      • Corn – December 10th
      • Grain Sorghum – December 10th
      • Cotton – December 31st

**Please Note: Cotton stalks must remain intact for the AIP’s inspection. The stalks must not be destroyed until the earlier of the AIP’s inspection or 15 days after harvest of the balance of the unit is completed.
